You must be between the ages of 16-21 Student who 18 years of age must be accompanied by a mentor, aunt, uncle, etc. (who must be at least 21 years of age) You must be accompanied by a parent/guardian, or advocate |
Bring the following information with you:
| 1. Transcript with test scores and service learning hours.
2. Up-to-date immunization record. 3. Birth Certificate. 4. Proof of Address.(telephone bill, gas and electric bill, lease or mortgage only) |
